Bursa Uludağ Üniversitesi Tıp Fakültesi Hastanesi’nde Hepatit E Virüsü Seroprevalansının Araştırılması

Abstract

Bu çalışma, Türkiye’nin Güney Marmara Bölgesi’nde yer alan üçüncü basamak bir sağlık kuruluşuna başvuran hastalarda Hepatit E Virüsü (HEV) seroprevalansını belirlemeyi ve bölgenin epidemiyolojik durumunu değerlendirmeyi amaçlamıştır. Mayıs 2023–Haziran 2025 tarihleri arasında 288 serum örneği retrospektif olarak analiz edilmiştir. Anti-HEV IgM ve IgG antikorları ELISA yöntemiyle ölçülmüş ve veriler istatistiksel olarak değerlendirilmiştir. Anti-HEV IgG pozitifliği %1,7, anti-HEV IgM pozitifliği %2,1 olarak saptanmış; örneklerin %96,2’sinde her iki belirteç de negatif bulunmuştur. IgM pozitif hastalarda akut HEV enfeksiyonu doğrulanmamıştır. IgM pozitifliği saptanan çocuk hastaların çoğunda hematolojik hastalıklar ile Epstein-Barr virüsü veya parvovirüs ko-enfeksiyonları bulunması, olası çapraz reaktiviteyi düşündürmüştür. Güney Marmara Bölgesi’nde HEV prevalansının düşük olduğu görülmüştür. Serolojik testlerin sınırlılıkları ve yalancı pozitiflikler nedeniyle, akut enfeksiyonun doğru tanısı için HEV RNA gibi moleküler yöntemlerin kullanılması gerekmektedir. HEV epidemiyolojisinin daha net ortaya konabilmesi için moleküler doğrulama yöntemlerini içeren daha geniş kapsamlı epidemiyolojik çalışmalara ihtiyaç vardır.

Investigation of Hepatitis E Virus Seroprevalence at Bursa Uludağ University Faculty of Medicine Hospital

Abstract

This study aimed to determine Hepatitis E Virus (HEV) seroprevalence in patients at a tertiary center in Southern Marmara, Türkiye, and evaluate regional epidemiology. A retrospective analysis of 288 serum samples (May 2023–June 2025) was performed. Anti-HEV IgM and IgG were measured by ELISA, and data were statistically analyzed. Anti-HEV IgG positivity was 1.7%, anti-HEV IgM positivity was 2.1%, and 96.2% of the samples were negative for both markers. No acute HEV infection was confirmed in IgM-positive patients. Children with IgM positivity often had hematological diseases and Epstein-Barr or parvovirus co-infections, suggesting possible cross-reactivity. HEV prevalence is low in Southern Marmara. Due to serological test limitations and false positives, molecular methods like HEV RNA testing are needed for accurate acute infection diagnosis. Broader epidemiological studies incorporating molecular confirmation methods are recommended for a more definitive understanding of HEV status.
