The Reliability of Tuberculin Skin Test in Diagnosis of Tuberculosis and its Connection to the Some Parameters

Abstract

The tuberculin skin test (PPD) is the oldest and the most widely used one among all the immunological tests. Tuberculin skin test results should be evaluated within the context of each patient's epidemiologic and environmental potential for infection. The positive PPD is one manifestation of type IV delayed hypersensitivity againts the M.tuberculosis. But PPD may be positive with BCG vaccination or nontuberculous atypical mycobacteria. Furthermore, certain biologic conditions such as immunosuppressive disease or drugs can cause negative PPD. Thereby,we aimed to question the effectiviness of tuberculin skin test in diagnoses of tuberculosis. PPD results of 116 adult patients with active pulmonary tuberculosis and 108 patients with nontuberculosis and normal immunologic status judged by to radiologic and bacteriologic examinations were evaluated. The diameter of enduration in active pulmonary tuberculosis was statistically higher than those in healthy people. When we considered 5,10,15 mm for cut off values, the pair of sensitivity and specificity were %74-%10, %56-%45 and %18-%68, respectively. We concluded that PPD is an important diagnostic method for tuberculosis but starting the treatment of tuberculosis relied on only PPD result is unfavorable.
