US: Can It Replace Lateral Decubitus Roentgenography In The Differentiation Of Free And Loculated Pleural Fluid?
Abstract
The aim of this study was to evaluate the accuracy of US versus LDR to differentiate free and loculated pleural fuild. Patients with pleural fuild detected at US or/and chest X ray were included into the study. The posteroanterior, lateral and lateral decubitus radiograms (LDR) and thorax US examinations were obtained in all patients. US examinations were performed both in supine and erect position at the same intercostal space in axial plane. In the same time, thickness of the fluid was measured in axial plane. The difference between the thicknesses of the two positions was calculated. If the difference was over 1 cm., fluid was accepted as free. US and LDR were evaluated by two different radiologists whom unaware of each other. Twenty- eigth pleural fuild were evaluated in 23 patients. Seventeen were free and the rest was loculated in LDR. US results were concordant with LDR in all cases. US should be the method of choice in differentiation of free and loculated pleural effusion instead of LDR, since it is cheap, harmless and easily applicable especially in intensive care units.
