Venous Thrombectomy and Extraanatomic Bypass in A Patient With Delayed Femoral Arterial Occlusion and Deep Venous Thrombosis: Case Report

Abstract

Atherosclerotic arterial occlusion is a frequent cause of lower extremity ischemia. Although aorto-bifemoral bypass is considered to be the gold standard, it proceeds with high mortality and morbidity rates in elderly patients with existing problems. In this case, we present a 48 years-old patient with the complaints of leg ischemia and deep venous thromboembolism with late admission. Bilateral arterial embolectomy, right femoral endarterectomy, right axillo-femoral and right to left femoro-femoral bypass with synchronous venous thrombectomy were performed. Axillobifemoral bypass can be effectively performed as an alternative technique when the aortobifemoral bypass is not feasible due to co-morbidities.

Gecikmiş Femoral Arter Trombozu ve Derin Ven Trombozu Olan Bir Hastada Venöz Trombektomi ve Ekstra-Anatomik Bypass: Olgu Sunumu

Abstract

Arterlerin aterosklerotik tıkanmaları alt ekstremite iskemisinin sık görülen sebepleri arasındadır. Aorto bifemoral bypass tedavide altın standart olmakla birlikte özellikle yaşlı hastalarda varolan diğer sorunlar nedeniyle yüksek morbidite ve mortalite ile uygulanabilmektedir. Bu olguda 48 yaşında bir erkek hasta gecikmiş bacak iskemisi ve derin venöz tromboz bulgularıyla müracaat etti. Bilateral femoral arteriyal embolektomi, sağ femoral endarterektomiyi takiben sağ aksillo femoral ve femoro femoral bypass , eş zamanlı olarak venöz trombektomi yapıldı. Birlikte varolan sorunlar nedeniyle aorto bifemoral bypass yapılamayan durumlarda aksillo bifemoral bypass etkin bir alternatif olarak kullanılabilir.
