A Comparison of Propofol and Dexmedetomidine on the Effects of Heamodynamy and Cognitive Functions in Conscious Sedation during Endoscopic Retrograde Cholangiopancreatography

A Comparison of Propofol and Dexmedetomidine on the Effects of Heamodynamy and Cognitive Functions in Conscious Sedation during Endoscopic Retrograde Cholangiopancreatography

Abstract

We have compared the effects of propofol and dexmedetomidine hemodynamy and respiratory system, side effects, cognitive functions, patient and surgeon satisfactions during endoscopic retrograde cholangiopancreatography (ERCP) for conscious sedation in the patients who had receive either propofol or dexmedetomidine. Fifty patients, ASA I and II, between the ages of 18-80 were included in the study. Demographic data and vital signs of the patients were recorded during the study. Ramsay sedation scale (RSS) was used to assess the level of sedation. Patients were randomly assigned into two groups; Group P received propofol 75µg kg-1 hr-1 intravenously (iv) over 10 minutes, followed by an infusion of 12.5-100.0 µg kg-1min-1 to achieve a RSS of 3-4. Group D received dexmedetomidine 1µg kg-1 hr-1 i.v over 10 minutes, followed by an infusion of 0.2-0.7 µg kg-1 hr-1 titrated to achieve a RSS of 3-4. End of the procedure, infusion of drug was stopped. A mini mental status examination (MMSE) was performed prior to sedation and in the recovery room when the modified Aldrete score (MAS) was 9-10. Pain was evaluated using the faces pain scale. Heart rate (HR) was lower in Group D at the 5th, 10th, 15th, 20th, 25th, 30th, 35th and 40th minutes following the sedation (p<0.001). HR was lower in Group D compared with Group P (p<0.05). Arterial blood pressure values were significantly lower in Group D compared with baseline (p<0.05). There was no difference in patient and surgeon satisfaction between the two groups. MMSE were statistically higher in Group D at the recovery room (p<0.05). Dexmedetomidine could be an alternative to propofol for conscious sedation during short invasive procedures such as ERCP.
