Comparison of Hemodynamic Responses to Chest Physiotherapy and Aerobic Exercise Training Before and after the Treatment in Children with Cystic Fibrosis

Abstract

This study was designed to evaluate the hemodynamic responses to chest physiotherapy and aerobic exercise training in children with cystic fibrosis. Sixteen patients with cystic fibrosis between the ages 5-14 years were included in this study. All patients were mild and modarete according to their Shwachman clinical scores and Crispin and Norman radiologic scores. All subjects received chest physiotherapy and aerobic exercise training on treadmill three days a week for six weeks. Spirometric tests, oxygen saturation levels, chest expansion, cardiovascular endurans, hearth rate and blood pressure values were evaluated before and after the treatment. By evaluating the results of the treatment, there was an increase in % VC, % FEF 25-75 and decrease in % FEV1, %FVC, and % PEF which were statistically significant (p<0.05). The oxygen saturation values were increased after the treatment (p<0.05). Heart rate, blood pressure, and double product values were decreased after the treatment. The chest expansion was improved significantly after the teratment (p<0.05). It is concluded that in addition to medical approaches, the active cycle of breathing techniques along with the aerobic exercise training helps to enhance the aerobic performance, and improves pulmonary parameters and hemodynamic responses in cystic fibrosis.

Göğüs Fizyoterapisi ve Aerobik Egzersiz Eğitimi Programı Uygulanan Kistik Fibrozisli Çocukların Tedavi Öncesi ve Sonrası Hemodinamik Yanıtlarının Karşılaştırılması

Abstract

Bu çalışma göğüs fizyoterapisi ve aerobik egzersiz eğitimi uygulanan kistik fibrozisli çocukların hemodinamik yanıtlarını değerlendirmek amacıyla planlandı. Çalışmaya toplam 16 olgu dâhil edildi ve olgular, yaşları 5-14 yıl arasında değişen, kistik fibrosis teşhisi konmuş ve Shwachman klinik skorları ve Crispin ve Norman radyolojik skorlarına göre orta ve hafif şiddetteki hastalardı. Olgulara 6 hafta süre ile haftada 3 gün olmak kaydı ile aktif solunum teknikleri döngüsü ve koşu bandında egzersiz eğitimi uygulandı. Tedavi öncesi ve tedavi sonrası, solunum fonksiyon testleri, oksijen saturasyon değerleri, göğüs çevre ölçümleri, kardiyovasküler enduransları, kalp hızı ve kan basıncı değerleri değerlendirildi. Tedavi sonunda yapılan değerlendirmelerde solunum fonksiyon testi parametrelerinden % VC ve % FEF 25-75’te artışlar görülürken % FEV 1, %FVC ve % PEF değerlerinde düşüşler görüldü (p<0.05). Oksijen saturasyon değerlerinde tedavi sonrası artış görüldü (p<0.05). Kalp hızı, kan basıcı ve hız-basınç çarpım değerlerinde düşüşler görüldü. Olguların göğüs çevre ölçümlerinde anlamlı artışlar görüldü (p<0.05). Kistik fibrozisli olgularda tutulan sistemlere yönelik uygulanan tıbbi yaklaşım ve medikasyonların yanı sıra, solunum egzersiz teknikleri ile birlikte uygulanan aerobik eğitimin, aerobik performansı arttırdığı, hemodinamik yanıtları ve solunum parametrelerini geliştirdiği görüşüne varıldı.
