Retrospective Analysis of Etiologic and Prognostic Factors of Patients Diagnosed with Acute Pancreatitis

Abstract

The aim of study was to assess the etiological factors and complications of the patients who were followed with diagnosis of acute pancreatitis (AP). A total of 184 patients who were diagnosed with AP in our hospital between the dates 2005-2010 were assessed retrospectively. Demographic characteristics, laboratory findings, imaging modalities, disease severity, prognosis, length of hospital stay of each case were recorded. The relation between severity of the disease with etiological factors, age, sex, length of hospitalization and prognosis were evaluated. The number of male patients were 79 (42,9%) and the number of female patients were 105 (57,1%) and the mean age was 55,5±17,01. Acute biliary pancreatitis was detected in 112 (60,9%) patients. Severe pancreatitis was observed in 50 (27,2%) patients. The mortality rate was 3,8%. There was no relation between the demographic characteristics of the patients and laboratory findings with severity of the disease. The mortality rate and length of hospitalization was higher in the severe group. AP is an important clinical entity which may cause mortality. Several etiological factors are responsible for development of AP, however biliary stones are the leading cause. Computed tomography severity index is an accurate predictor of severity and prognosis.

Akut Pankreatit Tanılı Hastaların Etyolojik ve Prognostik Faktörlerinin Retrospektif İncelenmesi

Abstract

Bu çalışmada akut pankreatit (AP) tanısı ile takip edilen hastaları etiyoloji ve komplikasyonlar açısından değerlendirmeyi amaçladık. 2005-2010 tarihleri arasında hastanemizde AP tanısı alan 184 hasta geriye dönük olarak incelendi. Olguların demografik özellikleri, laboratuvar bulguları, görüntüleme sonuçları, hastalık şiddeti, prognoz ve hastanede yatış süresi kaydedildi. Hastalık şiddeti ile etyolojik faktörler, yaş, cinsiyet, yatış süresi ve prognoz arasındaki ilişki incelendi. Erkek hastaların sayısı 79 (%42,9), kadın hastaların sayısı 105 (%57,1) ve ortalama yaş 55,5±17,01 idi. Yüz on iki olguda (% 60,9) akut biliyer pankreatit saptandı. Şiddetli pankreatit 50 olguda (%27,2) gözlendi. Mortalite oranı % 3,8 (7 olgu) bulundu. Hastalık şiddeti ile demografik özellikler ve laboratuar bulgular arasında ilişki saptanmadı. Mortalite ve yatış süresi şiddetli grupta daha yüksekti (p< 0,001). AP ölümcül seyredebilen önemli bir klinik sorundur. Etyolojisi çok çeşitli olup en sık neden safra kesesi taşıdır. Şiddet ve prognozun belirlenmesinde bilgisayarlı tomografi şiddet indexi uygun bir göstergedir.
