Incidental Placental Site Nodule In The Cervix Uteri

Abstract

Placental site nodules occur in the reproductive age group and are typically incidental findings in uterine curettage or cervical biopsy speci-mens and occasionally in hysterectomy specimens. The cells in PSNs look alike the intermediate trophoblasts and should differentiate the epithelial and mesenchymal lesions. PSNs can be found in the endocervix, in the endometrium, and rarely in the fallopian tube. A 42-year-old woman underwent hysterectomy for menorrhagia and metrorrhagia. There was a 0,8x0,5 cm in diameter nodular, brownish lesion on the lower uterine segment. On microscopic examination a nodular lesion with rounded, well-circumscribed borders, surrounded by a thin rim of chronic inflammatory cells was seen. The lesion were characterized by an abundant hyalinized extracellular matrix that separates tropho-blastic cells. The cells showed diffuse cytokeratin, and focal hPL and PLAP positivity immunohistochemically. The differential diagnosis between placental site nodule and cervical squamous cell carcinomas is particularly difficult in cervical biopsy specimens from patients who are being evaluated for cervical neoplasia. This should be kept in mind.
