Comparison of Normal and Extended Release Form Metformın in Term of Gastroıntestinal Side Effects in Type 2 Diabetic and Pre-Diabetic Patients

Abstract

Metformin is usually well-tolerated and commonly used oral-antidiabetic in type-2 diabetes and pre-diabetes. The most common side effects are gastrointestinal ones. It is proposed that extended-release form of metformin has less gastrointestinal side effects. We aimed to compare the extended-release and standard form metformin in type-2 diabetic and prediabetic patients for gastrointestinal tolerability, weight effect and glycemic control. Seventy-three newly diagnosed type-2 diabetic or prediabetic patients enrolled in the study. Standard form (group I) and extended-release (group II) metformin were started to patients with randomisation. Gastrointestinal symptom rating scale was used at the beginning and in the first month of treatment for evaluation. Gastrointestinal symptom scores were similar in both groups. At least one new symptom were found 20.5% of group I and 26.4% of group II. There was a slight increase in symptoms associated diarrhea in both groups and indigestion in the group II. No patients discontined treatment due to side effects. Weight loss was observed 4.0 and 3.0 kg for prediabetics in the group I and II, 2.5 kg for diabetic patients in the both groups. In diabetic patients, HbA1c reduced 1.28% and 1.24% in the group I and II. In conclusion, both forms are effective and similar in terms of gastrointestinal side effects.

Tip 2 Diyabetik ve Prediyabetik Hastalarda Metforminin Uzatılmış Salınımlı Formu ile Normal Formunun Gastrointestinal Yan Etkiler Açısından Karşılaştırılması

Abstract

Metformin tip 2 diyabetik ve prediyabetik hastalarda yaygın olarak kullanılan genellikle iyi tolere edilen bir oral antidiyabetiktir. En sık görülen yan etkileri gastrointestinal sistem yakınmalarıdır. Uzatılmış salınımlı formunun gastrointestinal sistem yan etkilerinin daha az olduğu öne sürülmektedir. Çalışmamızda metforminin uzatılmış salınımlı ve standart formunun tip 2 diyabetik ve prediyabetik hastalarda kullanımının gastointestinal tolerabilite, kilo ve glisemik kontrol üzerine etkilerini karşılaştırmayı amaçladık. Çalışmaya yeni tanı almış tip 2 diyabetik ve prediyabetik 73 hasta alındı. Hastalara rastgele randomizasyonla standart metformin (grup I) ve uzatılmış salınımlı metformin (grup II) başlandı. Tedavi başlangıcı ve bir ay sonra yapılan değerlendirmede gastrointestinal semptom derecelendirme ölçeği kullanıldı. Her iki gurupta gastrointestinal semptom skorları benzer bulundu. En az bir yeni yakınma oranı grup I'de %20.5 grup II'de %26.4 bulundu. Her iki grupta diyare ve grup II'de ayrıca hazımsızlık ile ilişkili semptomlarda hafif artış mevcuttu. Yan etki nedeniyle tedaviyi bırakan hasta olmadı. Grup I ve grup II'de prediyabetik hastalarda medyan 4.0(min-maks) ve 3.0(min-maks), diyabetik hastalarda ise her iki grupta med-yan 2.5(min-maks) kg kilo kaybı gözlendi. Diyabetik hastalarda HbA1c değerlerinde grup I'de %1.1, grup II'de %1.0 medyan düşüş saptan-dı. Bu sonuçlarla çalışmamızda her iki formun diyabet ve prediyabetiklerde etkin ve gastrointestinal yan etkiler açısından benzer olduğu görülmüştür.
