Abstract
Coronary arter perforations is a rare, but life-threatening complication of percutaneous coronary intervations. For coronary arter perforations, Ellis classification is used. The rate of death in patients with Ellis class 3 perforation has been about 44%. A 47-years old man with a history of coronary artery by-pass was hospitalised with a diagnosis of unstable angina. The Ellis class 3 perforation following routine stenting of saphenous vein graft stenosis was successfully treated by a polytetrafluoroethylene-covered graft stent. The coronary perforation following percutaneous coronary interventions should not be forgotten. İn addition to the availability of covered graft stents, it is essential to be familiar with various skills necesary for successful management of these complications.
Abstract
Greft Kaplı Stent İle Tedavi Edilen Safen Ven Grefti Perforasyon Olgusu Koroner arter perforasyonu perkütan koroner girişimler sırasında gelişebilen nadir fakat hayatı tehdit edebilen bir komplikasyondur. Koroner perforasyonlar için Ellis sınıflandırması kullanılır. Ellis evre 3 koroner perforasyon gelişen hastalarda ölüm oranı %44'lere kadar varmaktadır. 47 yaşında koroner by-pass öyküsü olan erkek hasta anstabil angina ile klinğimize yatırıldı. Safen ven greftine stent implantasyonu sırasında oluşan Ellis evre 3 perforasyon politetrafloroetilen kaplı stent ile başarılı bir şekilde tedavi edildi. Perkütan koroner girişimler esnasında koroner rüptür ile karşılaşılabileceği unutulmamalıdır. Bu nedenle koroner perforasyon tedavisinin iyi bilinmesi ve kateter laboratuvarında gerekli donanımın tüm işlem sırasında hazır bulundurulması gerekmektedir.
