IgA Nephropathy Patient with Staphylococcus Aureus Bacteremia and Cephalosporin Toxicity

Abstract

IgA nephropathy is one of the most common glomerulopathies worldwide. It may be seen as primary glomerulopathy and as secondary glomerulopathy depending on hepatobiliary diseases, autoimmune diseases, bacterial, viral infections and neoplasia. 15-40% of the patients have renal dysfunctions leading to end stage renal failure in 10 or 20 years. In the treatment of IgA nephropathy angiotensin converting enzyme inhibitors (ACE inh.), angiotensin receptor blocker (ARB) drugs, hydroxymethylglutaryl (HMG) CoA reductase inhibitors and immunsuppressive therapy are used. Resistant to immunesuppressive treatment and relapse after renal transplantation may be seen. Infection risk in chronic renal failure patients who get replacement therapy after catheterisation is high. S. aureus is one of these infection agents. Septic embolism, deep tissue abscesses, infective endocarditis, septic trombophlebitis etc. are common after S. aureus bacteriemia. In our case we presented septic brain embolism after S. aureus infection and encephalopathy caused by cephalosporine medication in chronic renal failure patient with IgA nephropathy.

IgA Nefropatili Hastada Staphylococcus Aureus Bakteriyemisi ve Sefalosporin Toksitesi

Abstract

IgA nefropatisi dünya genelinde en sık görülen glomerülonefritler arasında yer almaktadır. Hastalık primer glomerülopati olarak ortaya çıkabileceği gibi hepatobiliyer, otoimmün hastalıklar, bakteriyel ve viral enfeksiyonlar veya neoplaziler gibi sekonder nedenlere bağlı da ortaya çıkabilmektedir. Hastaların % 15-40'ı 10-20 yıl içinde son dönem böbrek yetmezliğine gidecek şekilde, renal disfonksiyonla kendini gösterebilir. IgA nefropatisi tedavisinde omega yağ asitleri, anjiotensin dönüştürücü enzim inhibitörleri (ACE inh.) veya anjiotensin reseptör blokörleri (ARB), hidroksimetil glutaril (HMG) CoA redüktaz inhibitörleri ve immünsüpresif tedavi yer almaktadır. İmmünsüpresif tedaviye direnç oluşabilmekte, renal transplantasyona rağmen hastalık nüks edebilmektedir. Kronik renal yetmezlik gelişen hastalarda renal replasman tedavisinde kateterizasyon sonrası enfeksiyon riski yüksek olup bunlar arasında S. aureus ta yer almaktadır. S. aureus bakteriyemisi sonrası septik emboli, derin doku abseleri, infektif endokardit, septik tromboflebit vb. riski yüksektir. Olgumuzda IgA nefropatisi bağlı kronik renal yetmezlikli hastada gelişen S. aureus enfeksiyonuna sekonder septik kraniyal embo-li ve sefalosporine bağlı ensefalopati gelişimi sunulmuştur.
