Dosimetric Comparison of Three Dimensional Conformal Radiotherapy, Volumetric Arc Therapy and Intensity Modulated Radiotherapy Techniques For Metastatic Servical Vertebra Radiotherapy: A Phantom Study

Abstract

The aim of this study is to examine the best treatment technique for metastatic servical vertebra in terms of dosimetry. A randophantom was used in this study. The computerized tomography image of the phantom was taken and used for treatment planning. On this CT image, a 3DCRT plan, three VMAT plans and four IMRT plans were calculated for servical vertebra. To calculate 3D-CRT plan, CMS XiO Treatment Planning System and to calculate VMAT and IMRT plans, Monaco Treatment Planning System were used. At the end of this study, treatment plans were compared by target volume (CTV) and critical organ doses, homogenity, conformity, MU and maximum treatment doses. According to results of the study, critical organ doses, MU and maximum treatment doses are better in 3D-CRT technique and homogenity and conformity indices are better in VMAT and IMRT technique plans. As a conclusion of this study, doses of organs at risk is better in 3DCRT plans and dosimetric properties of CTV are better in IMRT and VMAT plans.

Metastatik Servikal Vertebra Radyoterapisinde Üç Boyutlu Konformal Radyoterapi, Volümetrik Ark Terapi ve Yoğunluk Ayarlı Radyoterapi Tekniklerinin Dozimetrik Açıdan Karşılaştırılması: Fantom Çalışması

Abstract

Bu çalışmanın amacı, servikal vertebra radyoterapisinde farklı tedavi planlama yöntemlerinin kullanılmasının, dozimetrik olarak sebep olacağı avantaj ve dezavantajları tespit etmektir. Bu çalışmada, rando fantom (insan dokusu ile eşdeğer maket vücut) kullanılmıştır. Rando fantomun bilgisayarlı tomografi görüntüsü üzerinde; servikal vertebra için bir adet 3B-KRT, üç adet VMAT ve dört adet YART tedavi planı hesaplanmıştır. 3B-KRT planı için CMS XiO Tedavi Sistemi; YART ve VMAT tedavi planları için ise Monaco Tedavi Planlama Sistemi kullanılmıştır. Hazırlanan tedavi planları kritik organ ve CTV dozları, homojenite, konformite, Monitor Unit (MU) ve maksimum plan dozu açısından karşılaştırılmıştır. Çalışmamızda kritik organ dozları, MU ve maksimum plan dozu bakımından 3B-KRT tekniği ile en iyi sonuçlar elde edilirken, homojenite ve konformite bakımından YART ve VMAT tedavi planlarında daha iyi sonuçlar elde edilmiştir. Çalışmada elde edilen verilere göre, riskli organ dozları 3B-KRT tedavi planlamalarında daha iyi iken, YART ve VMAT tedavi planlamalarında CTV dozimetrik özellikleri, konformal tedavi planlarına göre daha iyidir.
