The Impact of the Quantitative Evaluation of Blastocyst Quality on Clinical Pregnancy Success

Abstract

Today, morphological parameters of blastocyst that determine live birth, chances of pregnancy and implantation are still controversial. The relationship between morphological parameters of embryo and clinical outcomes after embryo transfer is not yet fully understood and there is no consensus. In our study, we aimed to evaluate the effectiveness of some quantitative morphologic parameters (blastocyst and blastocyte diameter and area, trophectoderm cell number and inner cell mass area) that could be utilized objectively in selecting good quality embryos for implantation and clinical outcome. The main finding of our study was that the rate of ongoing implantation and clinical pregnancy was associated with embryo quality score; blastocyst area/diameter; blastosol area/diameter and the number of TE cells. The results showed that the ICM area/diameter, ICM roundness index and ZP thickness were not effective on clinical pregnancy rates.

Blastosist Kalitesinin Değerlendirmesinde Kantitatif Yaklaşımın Klinik Gebelik Başarısına Etkisi

Abstract

Günümüzde canlı doğum, gebelik şansı ve implantasyon oranını belirleyici blastosist morfolojik parametreleri halen tartışmalıdır. Embriyo morfolojik parametreleri ile transferi sonrası klinik sonuçlar arasındaki ilişki henüz tam olarak anlaşılamamış ve fikir birliği oluşmamıştır. Bu amaçla iyi kalite embriyo seçiminde objektif olarak kullanılabilecek kantitatif bazı morfolojik parametrelerin implantasyon ve klinik sonuçları belirlemedeki etkinliğini değerlendirebilmek amacı ile; bu çalışmada blastosist ve blastosöl çevre ve alan hesaplamaları, trofoektoderm (TE) hücre sayısı ve iç hücre kitlesi (ICM) alanının implantasyon üzerine etkileri değerlendirilmiştir. Çalışmanın temel bulgusu blastosist transferi ardından, implantasyon ve sonrasında devam eden klinik gebelik oranının; embriyo kalite skoru, blastosist alan ve çevresi, blastosöl alanı, çevresi ve TE hücre sayısı ile ilişkili olduğudur. Embriyo ICM alan ve çevresi, dairesellik indeksi ve ZP kalınlığının, klinik gebelik oranları üzerinde etkin olmadığı sonucuna varılmıştır.
