Our Cases of Proparacaine HCl Associated Keratopathy

Abstract

The purpose of the study is to present the clinical characteristics of patients abusing topical 0.5 % proparacaine HCL. The records of all patients with topical anaesthetic abuse referred to Department of Ophthalmology at Uludag University between May 2006 and February 2010 were reviewed. The study included 20 patients (20 males; age range: 19-49 years; mean age: 35 years). Sixteen of them were metal-workers. The causes for initiation of topical proparacaine HCl abuse was welding arc flash injury in 8 and corneal foreign body in 7 patients. The other causes for remaining 5 were insecticide spray exposure, foreign body strike, spectacle strike, keratitis and corneal ulcer. Mean time of abuse was 19 days (range 2-60 days). Epithelization time ranged from 4 to 60 days. Bandage contact lenses were used in all patients with 20% autologous serum and preservative –free dexamethasone drops. The patients were followed up for at least 6 months. Physicians should be familiar with the clinical characteristics and consequences of topical proparacaine HCL abuse.
