The Effects of Socio-economic Factors on the Development of Diabetic Retinopathy

Abstract

As a complication of diabetes mellitus (DM), diabetic retinopathy (DR) is the major cause of blindness in the working population. There are many factors which affect development of DR. We aimed to investigate the effect of socio-economic factors and HbA1C levels on the devel-opment of DR. In this study, 297 patients with DM were included and questionnaire about demographic and socio-economic status of pa-tients was performed. Patients were divided into two groups according to the presence or absence of retinopathy. The lower income and education, DM with duration of 15 years or more, high HbA1C levels and insulin treatment were all related to the development of diabetic retinopathy. Additionally, the questionnaire revealed that patients with DR were not informed about DR before and after the diagnosis of DM, and they were not regularly under control by a physician. As a result, both patient-related socio-economic factors and physician-related factors were found to be related to the development of diabetic retinopathy.

Diabetes Mellitus Hastalarında Sosyoekonomik Durumun Diyabetik Retinopati Gelişimine Etkisi

Abstract

Diabetes mellitus (DM) komplikasyonu olan diyabetik retinopati (DR), çalışan nüfusta en sık körlük nedenidir. Diyabetik retinopati gelişi-mini etkileyen birçok faktör mevcuttur. Bu çalışmada sosyoekonomik faktörlerin ve HbA1C değerinin retinopati gelişimine etkisini araştır-mayı amaçladık. Çalışmaya DM tanısı konulmuş 297 hasta dahil edildi. Hastalara demografik ve sosyoekonomik durumlarını sorgulayan anket uygulandı. Hastalar göz muayeneleri sonrası retinopatinin var olup olmamasına göre 2 gruba ayrıldı. Düşük gelir ve eğitim düzeyinin, 15 yıldan uzun diyabet süresinin, yüksek HbA1C değerinin, insülin kullanımının DR gelişimi ile ilişkili olduğu bulundu. Ayrıca retinopatisi olan hastaların, DM tanısından önce DRyi bilmedikleri, tanı aldıktan sonra DRnin kendilerine anlatılmadığı ve bu hastaların düzenli göz kontrolüne gitmedikleri tespit edildi. Bu bulgular, hem hastayla ilişkili olan sosyoekonomik faktörlerin, hem de hekimle ilişkili hastaların bilgilendirilmesi gibi faktörlerin diyabetik retinopati gelişiminde etkili olduğunu göstermektedir.
