Hyperchloremic Metabolic Acidosis Occur After Ureterosigmoidostomy: A Case Report

Abstract

Intestine is used in urological surgery for bladder augmentation as well as for ureter and genital reconstruction purposes. These operations are successfully performed in young patients as well as in elderly patients through modern reconstructive techniques. Many complications may however occur during post-operative period. Electrolyte disturbances, abnormal drug metabolism, infection, stone formation, nutrition disturbances, growth disturbances, osteomalacia and cancer development are among these complications. In this case report, we present a patient at the age of 30 with history of cystectomy and ureterosigmoidostomy performed at the age of 3. he has been diagnosed having hypokalemic hyperchloremic metabolic acidosis and he is presented to us with complaint of muscle weakness and numbness lasting since 1 month of age.

Üreterosigmoidostomi Sonrası Hiperkloremik Metabolik Asidoz: Bir Olgu Sunumu

Abstract

Ürolojik cerrahide bağırsak; mesane augmentasyonu, büyütme ve aynı zamanda üreter ve genital rekonstrüksiyon amaçlı kullanılmaktadır. Modern rekonstrüktif teknikler sayesinde genç hastaların yanı sıra, ileri yaşlı hastalarda da bu operasyonlar başarıyla gerçekleştirilmektedir. Operasyon sonrası dönemde çok sayıda komplikasyon görülebilir. Elektrolit bozuklukları, anormal ilaç metabolizması, enfeksiyon, taş oluşumu, beslenme bozuklukları, büyüme bozuklukları, osteomalazi ve kanser gelişimi bu komplikasyonların başında gelmektedir. Bu olgu sunumunda, 3 yaşında sistektomi ve üreterosigmoidostomi yapılmış, son 1 aydır kas güçsüzlüğü ve uyuşma şikayeti ile başvuran 30 yaşında hipokalemik hiperkloremik metabolik asidoz tanılı bir hasta sunmaktayız.
