YOĞUN BAKIMDA HEMŞİRELERİN KULLANDIĞI HASTA DEĞERLENDİRME YÖNTEMLERİ: TEKNOLOJİ VE FİZİKSEL MUAYENE KULLANIMI

Abstract

 Yoğun bakım üniteleri, hastalara yaşam desteği sağlayan, yüksek teknolojik cihazların kullanıldığı, hastaların sürekli izlem ve değerlendirmesinin yapıldığı özelleşmiş alanlardır. Her geçen gün teknolojideki ilerleme ve yenilikler hemşirelik bakımını ve hastaya dokunmayı etkilemekte, fiziksel muayene kullanımını gölgede bırakmaktadır. Bu bağlamda makale hastanın tedavi ve bakımında gerekli olan teknolojinin kullanımı ile birlikte fiziksel muayeneye öncelik verilmesi konusunda farkındalığı arttırmak amacıyla yazılmıştır. Yoğun bakım ünitelerinde hasta değerlendirmesi ve izleminde teknolojik cihazların kullanımı oldukça yaygındır. Literatürde hastanın hayati parametrelerinin izlemi ve değerlendirilmesi amacı ile teknolojinin doğru, zamanında ve amaçlı kullanımının bakımın kalitesini, hasta güvenliğini, hemşirenin otonomisini ve güvenini artırdığı; fakat cihazların kullanımı ve elde edilen bilgilerin doğruluğu ile ilgili birtakım sorunların yaşandığı belirtilmektedir. Yoğun bakım hemşirelerinin fiziksel muayene ile değerlendirdikleri bulgular çoğunlukla vücut sıcaklığı, nabız, solunum, kan basıncı, boy ve kilo ölçümü ile sınırlıdır. Bu parametrelerinin teknolojik cihazlar ile daha hızlı ve kolay ölçümü hemşireleri teknoloji kullanımına yöneltmektedir. Üstelik giderek artan bu eğilim hastanın durumu stabilize olduktan sonra bile hastanın izlem cihazlarına bağımlı kalmasına neden olmaktadır. Literatürde yoğun bakım hemşirelerinin fiziksel muayene yöntemlerini diğer kliniklere göre daha sık kullandığı ancak uygulamaların amaçsız ve eksik olduğu belirtilmektedir. Literatürde yoğun bakım ünitelerinde hastayı değerlendirme sürecinde teknolojinin ve fiziksel muayenenin kullanıldığı ancak teknolojinin daha çok kullanıldığı görülmektedir. Teknolojik cihaz ve fiziksel muayene kullanımının hasta bakımına önemli faydaları bulunmaktadır. Bundan dolayı yoğun bakım ünitelerinde her iki yöntemin birlikte kullanılması önemlidir.
