The value of IL-6 in the diagnosis of neonatal sepsis

Abstract

Objective: To determine the value of interleukin-6 in the diagnosis of neonatal sepsis. Materials- Methods: 70 newborn were included to the study. And divided into 4 groups. Group I: cases with positive blood cultures, Group II: cases having abnormal hematologic values with at least 2 clinical findings but on the other side having negative blood cultures. Group III: cases reminding clinical sepsis, having negative blood cultures and having another different diagnosis according to the results of the investigations. Group IV: control group. The hematologic values, IL-6 level, CRP values and blood cultures are eveluated in the study group when the first clinical sepsis finding is seen. Results: The IL-6 levels were found significantly high when group I (n=15, mean values=352,61 pg/ml) and group II(n=15, mean values=37,46 pg/ml) were compared with group III (n=20, mean values=4,19 pg/ml) and group IV (n=20, mean values=3,19 pg/ml). The mean values of IL-6 of group I, and group II were found statistically difference (p0,05) between the mean values of IL-6 of group III and IV. The normal value of IL-6 in healthy newborns, compatible with immulite kits was 0-9,7 pg/ml in our study; in the group having sepsis (group I+II) the sensitivity of IL-6 levels was found %83,3, and the spesifity was found %90. Discussion: The IL-6 levels which is measured in the first few hours of neonatal sepsis, have a high sensitivity and spesifity in diagnose.

Abstract

Amaç: Neonatal sepsisin tanısında bir sitokin olan IL-6'nın değerini belirtmek. Materyal-Metod: Çalışmaya 70 term yenidoğan alındı. Çalışmaya alman yenidoğanlar 4 gruba ayrıldı. Grup I, kan kültürü pozitif olan olgular; grup II, kültürler negatif ancak en az 2 klinik bulgu ile birlikte anormal hematolojik parametreleri olan olgular; grup III, başlangıç klinik bulguları sepsisi düşündüren kan kültürleri negatif ve tetkikler sonucunda başka bir teşhis alan olgular; grup IV, kontrol grubu. Çalışma grubundan sepsis klinik bulguları ilk gözlendiğinde hematolojik parametreleri, IL-6 düzeyi, CRP düzeyi ve kan kültürleri değerlendirildi. Bulgular: IL-6 düzeylerinin grup I (n=15, ort. değer=352,61 pg/ml) ve grup H'deki (n=15, ort. değer=37,46 pg/ml) değerleri, grup III (n=20, ort. değer=4,19 pg/ml) ve grup IV'deki (n=20, ort. değer=3,19 pg/ml) değerlerle karşılaştırıldığında anlamlı olarak yüksek bulundu.
