Persistan Wheezingin Nadir Bir Nedeni: Pulmoner Arter Sling

Abstract

Giriş: Pulmoner arter sling sol pulmoner arterin sağ pulmoner arterden köken aldığı vasküler pir patolojidir ve genellikle yaşamın ilk haftası ile ilk yılları içinde semptomlar ortaya çıkmaktadır.

Olgu Sunumu: Sekiz aylık erkek çocuk çocuk kardiyoloji polikliniğine düzelmeyen hışıltı nedeniyle yönlendirildi. Hastanın hikâyesinden şikâyetlerinin doğumun birinci ayından itibaren mevcut olduğu öğrenildi. Hastada persistan wheezing tespit edilmesi nedeniyle kardiyak değerlendirme yapılması amacıyla tarafımıza yönlendirildi. Hastanın kardiyak muayenesinde mezokardiyak odakta 4/6 pansistolik üfürümü mevcuttu. Ekokardiyografide perimebranöz bölgede Ventriküler Septal Defekt saptanan hastanın, sol pulmoner arterinin sağ pulmoner arterden kaynak aldığı şüphelenildi. Hastanın kontrastlı Toraks Tomografisinde Pulmoner arter slinginin olduğu ve trakeanın bu bölgede 3 mm'ye kadar daraldığı izlendi.

Sonuç: Wheezing ile başvuran hastalarda şikâyetlerin başlama zamanı ve karakteri iyi sorgulanmalı. Persistan wheezing saptanan hastalarda kardiyak değerlendirme dikkatli bir şekilde yapılmalıdır.
