Global trends in drug-induced syndrome of inappropriate antidiuretic hormone secretion research: a bibliometric analysis
Abstract
Aims: Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a major cause of hyponatremia and is frequently associated with drug use, particularly in hospitalized and older patients. Despite the growing clinical importance of drug induced SIADH, a comprehensive overview of research trends and knowledge structures in this field is lacking. This study aimed to perform a bibliometric analysis of the global scientific literature on SIADH and drug-related hyponatremia. Methods: A comprehensive search was conducted in the Web of Science Core Collection database using the Topic field. Publications from database inception to December 31, 2025 were included. After applying predefined filtering criteria, a total of 821 articles and reviews were analyzed. Bibliometric analyses were performed using the Bibliometrix package in R and its Biblioshiny interface. Key indicators included annual scientific production, author and journal productivity, citation metrics, keyword analysis, and collaboration networks. Results: Scientific production demonstrated a steady increase over time, with a marked rise after the early 2000s. The United States emerged as the leading contributor and central hub of international collaboration. “Hyponatremia” and “SIADH” were the most frequently occurring terms, highlighting the core focus of the field. Keyword and thematic analyses revealed three major research domains: pathophysiology, diagnosis, and pharmacological management. Treatment-related terms, particularly tolvaptan, gained prominence in recent years, reflecting the growing clinical adoption of vasopressin receptor antagonists and a shift toward targeted therapeutic strategies. Conclusion: The findings indicate a growing research interest in SIADH, with an increasing emphasis on clinical management and drug-related mechanisms. Future studies should focus on international collaboration and individualized treatment approaches, particularly in high-risk populations such as older adults.
