Hemogram-derived inflammatory indices across electrophysiological subtypes of Guillain-Barré syndrome

Abstract

Aims: This study aimed to evaluate the relationship between complete blood count-derived inflammatory indices, including neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), and platelet-to-lymphocyte ratio (PLR), and electrophysiological subtypes and short-term clinical outcomes in patients with Guillain-Barré syndrome (GBS). Methods: We retrospectively reviewed 50 adult patients with electrophysiologically confirmed GBS who were hospitalized in the neurology inpatient unit and/or intensive care unit between 2023 and 2025. Patients were classified as acute inflammatory demyelinating polyradiculoneuropathy (AIDP), acute motor axonal neuropathy (AMAN), or acute motor-sensory axonal neuropathy (AMSAN). Miller Fisher syndrome and rare variants were excluded. NLR, MLR, and PLR were calculated from admission complete blood count parameters. These indices were compared across electrophysiological subtypes and according to symptom onset-to-admission interval, cranial involvement, and discharge outcome. GBS-DS and EGRIS scores were additionally evaluated according to treatment modality. Results: The mean age was 56.1±16.4 years, and 36 patients were male. Inflammatory indices differed significantly among electrophysiological subtypes, with the highest NLR, MLR, and PLR values observed in the AMSAN group (p=0.022, p=0.003, and p=0.038, respectively). Admission GBS-DS, discharge GBS-DS, and EGRIS scores differed significantly among treatment groups (p=0.003, p=0.024, and p=0.020, respectively); post hoc differences were observed between IVIG and combined IVIG plus plasmapheresis (PF) for GBS-DS scores and between IVIG and PF for EGRIS. Inflammatory indices were not significantly associated with cranial involvement or discharge outcome. Conclusion: Hemogram-derived inflammatory indices were associated with electrophysiological subtypes in GBS, with higher values observed in the AMSAN group. These findings should be considered exploratory and require confirmation in larger prospective studies.
