Epithelioid Smooth-Muscle Tumor of Vulva with Uncertain Malignant Potential: A Case Report

Öz

The smooth muscle tumors are rare lesions of the vulva that represents a group including the tumors of uncertain malignant potantial.In this paper a case of epithelioid smooth muscle tumor of uncertain malignant potantial of the vulva in a 16 year old girl was encountered.The lesion was a well circumscribed solitary mass measuring up to 14cm Microscopically the lesion was composed of moderately pleomorphic epithelioid cells. The mitotic count is variable,the most mitotic active area showed 4-5 mitosis in 10 high power fields with an avarage of 1 per 10 high power fields.Immunohistochemically the tumor is immunoreactive for smooth muscle markers.Due to the great tumor dimension,moderately pleomorphic epithelioid cells and immunohistochemical profile of the tumor, the final histopatologic diagnosis of epithelioid smooth muscle tumor of uncertain malignant potantial was made.The lump was excised with negative surgical margins. The follow up information was available for 18 months and there was no recurrence or metastasis. This case gives us the opportunity to review the clinical and pathologic features of this rare entitiy determining the pathologic malignant criteria.

Vulvanın Malign Potansiyeli Belirsiz Epiteloid Düz Kas Tümörü; Olgu Sunumu

Öz

Malignite potansiyeli belirsiz bir grup tümörü de içeren vulvanın düz kas tümörleri nadir tümörlerdir. Burada 16 yaşında bir kız çocuğunda vulvada lokalize malign potansiyeli belirsiz epiteloid düz kas tümörü sunulmuştur.Lezyon, en geniş olduğu yerde çapı 14 cm e ulaşan iyi sınırlı soliter kitle şeklindedir ve mikroskopik olarak çoğunlukla orta dereceli pleomorfik epiteloid hücrelerden oluşmaktadır. Mitotik aktivite değişken olup en yoğun olduğu yerde 10 büyük büyütme alanında( 10 BBA) 4-5 e kadar çıkarken ortalama 1 mitoz görülmektedir(1 mitoz/10BBA). İmmunohistokimyasal olarak tümörde düz kas belirleyicileri ile boyanma izlenmektedir.Tümör boyutunun güvenli sınırın çok üzerinde olması, tümörü oluşturan hücrelerin epiteloid morfolojili olup orta derecede pleomorfizm göstermesi ve tümörün düz kas belirleyicileri ile boyanması olguya nedeniyle malign potansiyeli belirsiz epiteloid düz kas tümörü tanısı verilmiştir.Güvenli cerrahi sınır ile eksize edilen olguda 18 aylık takip süresi sonrasında rekürrens veya metastaz saptanmamıştır. Vulvar düz kas tümörlerinde tümör boyutu( 5 cm.den büyük boyut), infiltratif sınır, atipi( orta/ ileri derecede), mitoz (10 BBA'da 5 veya daha fazla) ve nekroz, benign-malign ayrımında ve rekürrenslerin tahmin edilmesinde primer belirleyici faktörlerdir. Sözkonusu olgu, nadir bir antite olması nedeni ile lezyonun klinik ve patolojik özelliklerin tekrarına ve patolojik malign kriterleri gözden geçirme fırsatı vermektedir.
