İMPLANT DESTEKLİ PROTEZLERDE İMMEDİAT YÜKLEME, YUMUŞAK DOKU ŞEKİLLENDİRMESİ VE BİREYSEL ÖLÇÜ POSTU ÜRETİLMESİ; OLGU SUNUMU

Öz

İmplant destekli protezlerin uzun dönem başarısını etkileyen en önemli faktörlerden biri sert ve yumuşak dokunun yeterli miktarda ve uygun şekilde olmasıdır. İmmediat yükleme protokolü, hastanın kaybettiği estetik ve fonksiyonu en hızlı şekilde geri kazandırmayı amaçlamaktadır. Restorasyonların çıkış profili, implant üstü protezlerin estetiğini ve uzun dönem başarısını etkileyen önemli faktörlerden biridir. Günümüzde uygun bir çıkış profili yakalamak amacıyla kişiye özel iyileşme başlıkları ve geçici restorasyonlar sıklıkla kullanılmaktadır. Çıkış profilinin düzgün bir şekilde ölçüye aktarılması daimi protezin uyumlu olması için son derece önemlidir. Ancak standart ölçü postlarının silindirik yapısı çıkış profilini ölçüye yansıtmak için tam olarak yeterli değildir. Bu olgu sunumunda, hastanın diş eksikliği bulunan bölgesine implant uygulanmasını takiben immediat yükleme protokolü ile geçici restorasyon uygulanmıştır. Bu işlemden 4 ay sonra geçici restorasyon kopyalanıp bireysel ölçü postu üretilmiştir. Bu bireysel ölçü postu, daimi restorasyonun üretimi için kullanılmıştır. Birbirinden farklı çalışmalarda farklı tedavi protokolleri uygulanabilse de asıl amaç implant destekli protezlerde doğal dişlere yakın bir çıkış profili oluşturmak ve daimi protezleri bu çıkış profiline uygun şekilde üretebilmektir.

IMMEDIATE LOADING, SOFT TISSUE MANIPULATION AND MANUFACTURE OF INDIVIDUAL IMPRESSION POSTS IN IMPLANT SUPPORTED PROSTHESES: CASE REPORT

Öz

One of the most important factors affecting the long-term success of implant-supported prostheses is the adequate and appropriate form of hard and soft tissue. The immediate loading protocol aims to restore the lost aesthetics and function of the patient as quickly as possible. The emergence profile of the restorations is one of the important factors affecting the esthetics and long- term success of implant-supported prostheses. Today, individual healing caps and provisional restorations are frequently used in order to achieve an appropriate emergence profile. Proper transfer of the emergence profile to the impression is extremely important for the compatibility of the final restorations. However, the cylindrical structure of standard impression posts is not fully sufficient to reflect the emergence profile to the impression. In this case report, a provisional restoration was performed with an immediate loading protocol following the application of an implant to the patient’s missing tooth area. After 4 months of this procedure, the provisional restoration was copied and an individual impression post was produced. This individual impression post was used for the fabrication of the final restoration. Although different treatment protocols can be applied in different studies, the main purpose is to create an emergence profile close to natural teeth in implant supported prostheses and to produce final restorations in accordance with this emergence profile.
