Nadir Karşılaşılan Bir Endodontik Problem: Radiks Entomolaris

Öz

Mandibular birinci büyük azı dişlerinin distolingualinde konumlanmış, süpernumerer üçüncü kök ‘’Radiks entomolaris’’in görülme sıklığı etnik popülasyona bağlı olarak % 14.4 olarak belirlenmiştir. Radiks entomolaris de en sık karşılaşılan kanal konfigürasyonu Vertucci sınıf 1 tiptedir. Genel olarak cinsiyet farklılığı yoktur ve çift taraflı ortaya çıkar. Diğer köklere göre vertikal olarak en kısa köktür ve ileri derecede kurvatürlü bir yapıya sahiptir. Kök kanal tedavisi ihtiyacı olan pulpal patolojilerde, teşhisi için en uygun yöntem paralel teknikle mesialden 25°’lik bir açıyla çekilmiş periapikal radyografiler veya cone-beam bilgisayarlı tomografi CBCT ’dir. İkizkenar yamuk şeklinde açılmış giriş kavitesi kanal girişinin bulunmasını kolaylaştırır. Biomekanik preparasyon sırasında ve post boşluğu hazırlama işlemleri sırasında furkasyon seviyesine dikkat edilmesi furkal ve strip perforasyonların önüne geçilmesini sağlar. Bu derlemenin amacı, iki köklü mandibular birinci büyük azı dişlerinin major anatomik varyasyonu olan distolingualde konumlanmış üçüncü kök Radiks Entomolarisin prevalansının, morfolojik sınıflamasının ve anatomik özelliklerinin tartışılması; klinik teşhis ve tedavisinin incelenmesidir

Rarely Encountered An Endodontic Problem: Radix Entomolaris

Öz

Distolingual area of mandibular first molar teeth located that supernumerary root ‘’ Radix entomolaris’’ incidence was %14.4 and associated with certain ethnic populations. The most common canal configuration of distal roots was Vertucci type 1, respectively. Generally, no significant differences were observed in the prevalence of radix entomolaris according to gender and unilateral or bilateral occurrance. The root length of the radix entomolaris was shorter than other roots. During the treatment of pulpal pathologies that requiring root canal treatment, the best methods to diagnosis radix entomolaris are 25° mesial parallax periapical radiograph or cone-beam computed tomography CBCT . A trapezoidal shape access cavity is benefit to locate the orifice of the radix entomolaris. During the biomechanical preparation and post-space preparation clinicians should be aware of the variable furcation levels to avoid furcal and strip perforation. The purpose of this review was to conduct a literature review on the prevalance, morphologic classification and anatomic characteristic of the two-rooted mandibular first molar is a tooth with an additional distolingual and third root: major anatomical variant the Radix entomolaris
