Primary fracture healing by contınuous callus compression technique after fresh fractures

Abstract

A new modification of compression osteosynthesis which was designed for treatment of fresh fractures is described in this study. Right tibias of 16 rabbits were osteotomized subperiosteally and Ilizarov type external circular fixators adapted to the fragments. In the experimental group (n=8) compression was begun at the 12th day postoperatively and a total of 14 mm interlragmental compression was done. fn the control group (n=8) fragments were held in neutral position, without any axial movement during the postoperative period. This study was stopped at the end of the 7th postoperative week. Clinical, radiological (x-ray, CT). scintigraphical (dynamic and static) and histopathological examinations were perlormed in this study. In the experimental group fragment cooslidation was achieved at the end of the 3rd week. At the end of the study, fracture healing was seen without external callus by way of primary fracture healing in the experimental group. In the other group consolidation was seen at the 5th week with excessive periosteal new bone formation. In conclusion, in the compression group the fracture healing period was shortened by half compared to the group secondary fracture healing Primary fracture healing was significantly activated by the rigid and continuous compression technique. Statistical analysis supported histologic and radiological results (p<0.01 , S).
