Results of conservatively treated elbow dislocations

Abstract

Twenty-six elbow dislocations treated by closed reduction were evaluated clinically and radiographically after an average of 39.8 months (18 to 55 months). Average age at the time of injury was 25.5 (range, 12 to 65 years). 9 patients had associated fractures, the most frequent being a fracture of the medial epicondyle (6 patients). The immobilization period for pure dislocations was three weeks. Associated fractures were treated surgically in three patients. Despite the generally favorable results, 62% of the patients reported some symptoms on follow-up. The most frequent residual complaints were Iimitation of motion and feeling of weakness. Flexion contracture and Iimitation of flexion were documented in 50% and 58 % of the patients, respectively. Severe Iimitation of motion was noted in 7 patients (27%). The prognosis was worse in cases with associated fractures. Limitation of motion was more severe in patients over 18 years of age. Radiographs demonstrated myositis ossificans in 19% of the patients. An adult who had an elbow dislocation associated with a capitellum fracture 17 years ago was evaluated separately. He had severe osteoarthrosis. The results of the present study indicate that elbow dislocations are not benign injuries.
