The evaluation of results of proximal and distal osteotomy in the treatment of hallux valgus

Abstract

Between the years of 1987 and 1991 in the Department of Orthopaedics and Traumatology of SSK Istanbul Hospital we applied surgical treatment to the 60 feet of 58 patients with hallux valgus. The follow. up period of this group, containing 43 female and 15 mate patients, was 26 months The average age of the group was 39.5 (13-76) Results of the treatment have been evatuated in accordance with Glückert and et alls objective and subjective critera; 44 (73.3%) excellent 10 (16.6%) good and 6 (10.1%) poor results. On the 43 feet of 44 patients, 30° anomali anomali oblique distal metatarsal osteotomy without capsuloraphy has been performed by regional anesthesia (Lindgren.Turan operation). On the 16 feet of 15 patients, proximal metatarsal osteotomy, buniectomy, lateral of MP joint and abductor release were performed (Tachdjian operation). The results of Lindgren-Turan operation were; 36 (81.1%) excellent 6 (13.6%) good and 2 (4.6%) poor where as the results of Tachdjian operation were; 8 (53.3%) excellent 4 (26.6%) good and 4 (26.6%) poor. Therefore, we conclude that Lindgren- Turan operation is preferable to Tachdjian operation for patients with those hallux valgus do not have extreme artrotic change. Because this operation; -provides the application of regional anesthesia, -does not have any serious complication, -does not cause any post operational pains and stiffness, -is more tolerable both for the patients and the doctor

Halluks valgus tedavisinde proksimal ve distal metatarsal osteotomi sonuçlarının değerlendirilmesi

Öz

SSK istanbul Hastanesi Ortopedi ve Travmatoloji Kliniği 1987- l991 yılları arasında 58 hastanın 60 ayağına halluks valgus nedeniyle cerrahi tedavi yapıld. Hastaların 43'ü kadın, 15'i erkek olup, yaş ortalamaları 39,5 (13-76), ortalama takip süresi 26 ay idi. Sonuçlar Glückert ve arkadaşlarının (1984) objektlf ve subjektif kriterlerine göre değerlendirildi; 44 (%73,3) çok iyi, 10 (%16.6) iyi ve 6 (%10.1) kötü sonuç alındı. 43 hastanın 44 ayağına rejional anestezi ile kapsulorafi 30° oblik distal metatarsal osteotomi (Lindgren- Turan Op.); 15 hastanın 16 ayağına proksimal metatarsal osteotomi buniektomi MP eklem lateral ve abduktor gevşetmesi ve medial kapsulorafi (Tachdijan Op.) yapıldı. Lindgren- Turan operasyonu başarı oranı 36 (%81.8) çok iyi, 6 (% 13.6) iyi ve 2 (%26.6) kötü sonuç alınmasına karşın Tachdijan operasyonunda 8 (%53.3) çok iyi, 4 (%26.6) iyi ve 4 (%26.6) kötü sonuç alındı. Sonuç olarak aşırı artrozik değişiklik bulunmayan halluks valguslu hastalarda rejional anestezi yapılabilmesi, teknik kolaylığı, post-op ağrı, hareket kısıtlılığı yapmaması, önemli bir komplikasyonu olmaması ve hem hasta"hem de hekim için daha tolere edilebilir olması nedeniyle Lindgren- Turan operasyonun Tachdijan operasyonuna nazaran tercih edilebilir bir yöntem olduğunu düşünmekteyiz.
