Treatment of nonmetastatic osteosarcoma

Abstract

19 nonmetastactic, nonresectable and extremity Iocalised osteosarcoma patients were treated with cispfatinum and epirubicin combinationin preoperative period. Patients above 13 years ofd also rdeived radiation therapy between the first two cycles. All patients had a Iimb salvage operation. At the post operative periode they continued to receive adjuvant chemotherapy according to their tumor necrosis rate To patients whom tumornecrosis rate was < %90. weadded ifosphamid8 for three consecutivedays. In total1y they receeived 6 courses. After the primary treatment, necrosis rate wase %90 in 9 patients, between %60-90 in 9 patients and, < %60 in 1 patient. One patient died with febrife neutropenia, 2 more patients died with mitastatic desease 1 patient refused the adjuvant ch8motherapy. 2 patients received > 5 courses of chemotherapy and they wereac cepted as a protocol violation and, excluded from the study. In 4 patinets metastatic disease was experienced within 12-20 months. 2 patients are still continuing to receivetheir adjuvant chemotherapy. We think that this is an efective and Well tolerated regimen. Better results can be obtained with addition of ifosphamide to primary (neoadjuvant) chemotherapy.

Metastazsız osteosarkomların tedavisi

Öz

İnoperabl olan ve metastaz yapmamış 19 ekstremite yerleşimli osteosarkom olgusu, sisplatin ve epİrubisinden oluşan kombine kemoterapi ile ame1iyat öncesi dönemde tedavi edildi. 13 yaşın üzerindeki olgulara ilk iki tedavi arasında radyoterapi deuygulandı Bütün olgulara ekstremite koruyucu cerrahl uygulandı. Ameliyat sonrası, tümördeki nekroz oram dikkate alınarak, kemoterapiye devam edildi. Nekroz oram %90'ın altında olanlara ek olarak, ifosfamid verildi. Toplam 6 kemoterapi alan hastalar, izemeye alındı. Nekroz oram 9 olguda %90 ve üzerinde, 9 olguda %60-90 arasında, 1 olquda ise %60'ın altında bulundu. Bir hasta septisemi nedeniyle, iki hasta da metastatik hastallk nedeniyle kaybedildi. Bir hasta ame1iyat sonrası tedaviyi kabul etmediği için, iki tanesi, amefiyatı çok geciktiği için değer1endirmeye alınamadı. Toplam 4 hastada. metastaz gelişti, hiçbirinde fokal nüks görülmedi. 13 hasta (olgulann %76'sı), 5-33 aydır izlenmektedir, iki olgunun kemoterapisi sürmektedir. Uygugamakta olduğumuz tedavinin etkili olduğunu, neoadjuvant edavi sırasında tedaviye ifosfamidin de eklenmesiyle daha iyi sonuçlar elde edeceğimizi düşünmekteyiz.
