The use of holmium- Y AG laser in arthroscopy of the knee

Abstract

Holmium-YAG Iaser was used in 51 of 390 patients who underwent knee arthroscopy between March 1993-0ctober 1994. There were 31 male and 20 female patients and the mean age was 37 (12-69) The device was a near contact Holmium-YAG Iaser that emits infrared Iaser energy with a wavelength of 2100 nm, in a pulsed. transmitted by a fiberoptic cable. A total of 72 procedures; 32 partial meniscectomies, 31 chondral debridements, 2 partial synoviectomies. 3 plica excisions, 2 lateral releases and 2 osteophyte ablations were performed. The decision to use the Iaser was always made intraoperativeiy, when it was thought that conventional instruments were unsuitable for the procedure. There were no early complications related to Iaser. Postoperatively the patients required Iess pain medication and there were no significant effusions The thin Iaser probe was especially useful in tight knees, minimizing the risk of cartilage damage associated with Iarger mechanical instruments and in chondrallesions where the depth, contour and the extent of the debridement was easliy controlled. The haemostatic effect of Iaser was helpful in cases without a tourniquet and Iaser was safely utilized in patients undergoing arthroscopy under Iocal anesthesia in the out-patient setting. lt was concluded that, although the cost is still prohibilive, Iaser is an important tool that should be used in selected cases when the mechanical or motorized instruments are inadequate or diffucult to use. Effective precautions should be taken to protect the patient and operating room personnel from accidental exposure of Iaser
