Clinical and surgical approach to the calcifying tendinitis

Öz

Calcifying tendinitis is a polymorphous disease characterized by calcium hidroxyapatite crystals differing in shape, size and site. The condition is not unique to the shoulder, and similar Iesions are seen in tendons and Iigaments around the ankle, knee, hip and elbow. Calcium deposits in calcifying tendinitis are Iocalized inside a tendon. They are usually not in continuity with or extending in to the bone. The cause is unknown but it is thought that Iocal ischemia Ieads to fibrocartilaginous metaplasia and active shedding of crystals by the chondrocytes. Evolving slowly over a Iong time with a painful period whose duration is often difficult to forecast and a Iong term tendency to self- healing. We are going to discuss the treatment of calcifying tendinitis in 11 shoulders (right 5, Ieft 2, bilateral2) of 9 (female 6, male 3) patients treated in our clinic between January 1990 to December 1993. We made a single Iocal steroid and analgesic injection after the diagnosis and the patients were evaluated after one and three weeks and at three months after the injection. We performed surgery in three patients who were still painfull at three months after the injection. In conclusion Iocal steroid and analgesic injection seem to be more effective in patients who are in the early stages of the disease however surgery with a anterior transdeltoidal approach appears more effective in patients who are in the Iater stages of the disease.
