Instability after ankle fractures

In this study effects of the reduction of the fibular fracture and the distal tibio-fibular syndesmosis on tibio-talar joint were investigated. Discussion subjects, such as transsyndesmotic fixation, acceptable displacement of fibular fracture, the management of SE 2 type fractures, postoperatively external fixation and importance of syndesmotic calcification were reviewed under the light of our cases. Among the 70 patients 32 of them were evaluated by clinically and radiologically, after an average follow-up of 17 months. Based on the findings in our series, we concluded that adequate reduction of the syndesmosis is necessary to achieve a stable ankle joint after supination-external rotation, pronation-external rotation and pronation-abduction fractures. With the use of the suprasyndesmotic screw, weight-bearing was permissible with the screw in situ. As much as two millimeters of lateral displacement of lateral or medial malleolus is accessible with a satisfactory result. Slight or moderate calcification of the syndesmosis seems not to affect the end results considerably. If a stable reduction was achieved it is not necessary to use an external support or cast immobilization.
