Septic arthritis in the childhood

Abstract

35 patients with the provisional diagnosis of septic arthritis were admitted the 2nd Orthopaedic Clinic of ministry of health Ankara Hospital. 21 patients were male and 14 were female with an average age of 9.1 years (range 5 mo-15 yrs.). Septic arthritis was considered in the hips of 19 patients, in the knees of 12, on the shoulders of 3 and on the elbow of 1. All of the patients were operated on an emergency basis with drainage of pus and application of washout drainage. Wide spectrum antibiotics were administered postoperatively (3 week parenterally, 3 week orally). At the time of admission 94.2% of patients had high sedimentation rate, 60.0% of patients had high leucocyte count. Cultures revealed Staph. aureus in 24 cases, gram (-) bacteria in 6 (Enterobacter cloaca, E. coli, Coliform basil) and no growth in 5 cases. In patient with hip septic arthritis we have observed three femoral head destructions and one septic dislocation. In patients with three septic arthritis of the knee, we postoperatively have found two femoral osteomyelitis. As a result, it was seen that delay in the drainage of joints with septic arthritis caused many complications, early intervention has resulted with 90% success.

Çocuklarda sepitik artrit

Ocak 1993 ile Ocak 1995 tarihleri arasında S.B. Ankara Hastanesi 2. Ortopedi Kliniğine başvuran, septik artritli 35 çocuk hasta retrospektif olarak değerlendirildi. Hastaların 2’si erkek, 14'ü kiz idi ve ortalama yaşları 9,1 yıl (5ay-15 yaş) olarak saptandı. 19 hastada kalça septik artriti, 12 hastada diz, 3 hastada omuz ve 1 hastada dirsek septik artriti teşhis edildi. Hastaların hepsi acil olarak opere edildi. Drenaj ve yıkama sonrası postoperatif geniş spektrumlu antibiyotik başlandı (ilk 3 hafta paranteral, sonraki 3 hafta oral). Yapılan laboratuvar tetkiklerinde %94.2'sinde sedimantasyonun yüksek, %60.0'ında da beyaz kürenin yüksek olduğu saptandı. Operasyonda alınan kültürlerin 24'ünde staph. aureus, 6'sında gram (-) bakteri (enterobakter cioaca, E. coli. Coliform basil), 5 hastadan alınan kültürlerde ise üreme olmadı. Kalça septik artritli 3 hastada femur başı destrüksiyonu, 1 hastada septik dislokasyon saptandı. Diz septik artritli 2 hastada eksplore edilen diz ekleminin çevresinde postoperatif osteomyelit görüldü. Sonuç olarak septik artritli çocuklarda eklem drenajı geciktikçe, sekel ve kompiikasyon oranının arttığını, erken müdahalede ise %90 oranında başarı elde edildiğını gördük.
