Arthroscopy of the wrist

Abstract

From 1970s, development in arthroscopic equipment and techniques has extended the ability of large joint arthroscopy to be performed in smaller joints. The wrist is a complex system of articulations and ligaments. That's why, wrist arthroscopy began to gain widespread acceptance on the world only in the last ten years. Although the knee arthroscopy is performed well on many patients, wrist arthroscopy is attempted in small numbers and at few centers, in the Turkey. It provides the surgeon with a magnified view of all intraarticular structures, including those areas difficult to access through an arthrotomy. Arthroscopic surgery is minimally invasive, hence patients rehabilitate quickly with fewer complications. Therefore, wrist arthroscopy provides important diagnostic information and therapeutic intervention, while avoiding the significant morbidity of open techniques as the same advantages are afforded in large joint procedures. In this chapter on wrist arthroscopy will discuss why, when, and how to employ arthroscopic techniques to the wrist.

Öz

1970'li yıllarda ortopedik cerrahide kullanılmaya başlanan artroskopi ekipman ve tekniklerindeki gelişmeler zaman içinde büyük eklem artroskopilerindeki uygulama benzerlerinin küçük eklemlerde de yapılabilmesini mümkün hale getirmiştir. Kompleks bir eklem ve ligament yapısına sahip olan el bileğine yönelik artroskopi uygulamalarında dünyada son on yılda önemli gelişmeler olmuştur. Ülkemizde ise artroskobik yöntem ile yapılan tanısal ve cerrahi uygulamalar diz eklemi bölgesinde yoğunlaşmış, el bileği artroskopisı gereken düzeyine ulaşamamıştır. El bileği artroskopisi artrotomi ile bile ulaşılması güç olan bölgeler dahil olmak üzere eklem içi yapıların magnifikasyon altında görülmesine olanak veren minimal bir invazif girişimdir. Büyük eklem artroskopisindeki gibi hasta rehabilitasyonunun hızlandırılması ve daha az komplikasyon gibi avantajların oluşmasını sağlar. El bileği artroskopisinin neden, ne zaman ve nasıl uygulanacağının yer aldığı bu bölümde el bileğinin artroskopik anatomisi, giriş yolları, temel artroskobi tekniği, tanısal ve cerrahi artroksobi uygulamaları hakkında bilgi verilecektir.
