Cervical facet dislocation management following a delay in diagnosis

Abstract

The management of cervical facet dislocations (CFD) has been reported to be complicated and controversial. Several authors have suggested that the workup and subsequent treatment of these patients may be altered if there is a delay in their initial presentation. The objective of our study was to determine the influence of early versus late CFD presentation on patient management. All patients treated for CFD from January 1988 to December 1992 at The Ben Taub General Hospital Houston, Texas were identified for retrospective review. Patients were divided into two groups based on time of presentation to hospital Group 1 (Early)<72 hours, Group 2 (Late)> 72 hours. Charts were reviewed in detail to determine pre and post treatment neurological status and the reduction method (traction versus surgery) used. 34 patients with CFD were identified 22 patients. In Group 1, facet reduction was achieved closed in 64%, whereas in Group 2 facet reduction was achieved closed in only 20% patients. In our patients, 35 percent were late in obtaining treatment. Less neurological deficit was demonstrated in this group of patients and closed reduction was less likely to be successful. The authors conclude that a distinction should exist between early and late presentation in the management algorithm of CFD.
