A comparison of the free flaps to conventional procedures in terms of the lower extremity tissue defects

Abstract

Between 1987 and 1995, 66 patients who were treated with diagnosis of the complicated lower extremity tissue defect were evaluated by a retrospective study. In this study, the results of free flap transplantations and conventional methods were compared in points of the hospitalization time, number of general anesthesia, total cost, type, location and size of defect, and complications during early and late postoperative periods as well. Of 66 patients, 51 were treated with conventional methods, and 15 patients underwent reconstruction of lower extremities with free flap transplantation. The case number of conventional flaps that implies cross-leg flaps and flaps prepared from the same extremity was 27 and 24 respectively. As a result of these comparative study, flaps prepared from the same extremity is considered to be primary choice of treatment modalities in case of proper indication. And the free flap applications are recorded as superior to the cross-leg applications regarding the shorter hospitalization time, reconstruction with the one-stage operation, and the earlier mobilization chance; but inferior to all conventional methods regarding the higher cost, the longer operative time and the necessity of qualified team for microsurgical technique.
