Irreducible dislocation of the interphalangeal joint of the great toe following a soccer injury" A case report"

Abstract

We report a unique dislocation of an athlete' s great toe interphalangeal joint. The injury required surgical treatment to reduce an entrapped plantar sesamoid and allow relocation. The patient, a 19-year -old man, while he was playing soccer on ground courtyard with his synthetic grass court shoes, he has attempted to jump to the ball coming from low level when his forefeet touching on the ground. After this attack the great toe of his left foot had been folded backward and then he had not attend the play. Two days later from the injury he admitted to emergency department of our clinic. Through the clinical examination dorsal displacement of the distal phalanx at the interphalangeal joint of the left great toe has been observed. In the radiographic examination at the level of great toe interphalangeal joint we defined dislocation due to interposition of the sesamoid bone in the interphalangeal joint. After attempt of unsuccessful closed reduction we performed open reduction. Arthrotomy was performed with the dorsal incision and then sesamoid bone was reduced from interphalangeal joint and reduction was performed. After the operation below knee cast splint was used for 4 weeks. At the follow-up examination, 22 months later from the injury, the case was symptom free with normal range of motion of the injured toe without arthritic change in the interphalangeal joint of the left great toe.

Futbolda ayak baş parmağının interfalangeal ekleminin redükte edilemeyen çıkığı(*) (Olgu sunumu)

Öz

Bu yazıda sporcularda nadir görülen ayak baş parmağının interfalangeal eklentinin redükte edilemeyen çıkığıyla ilgili bir olgu bildirildi. Redüksiyona engel olan plantar sesamoidin eklem aralığından çıkartılması için cerrahi girişim gerekli oldu. 19 yaşındaki erkek hasta, toprak zeminli sahada sentetik elyaflı çim saha-larda oynamaya mahsus ayakkabıyla futbol oynarken alçak seviyeden gelen topa kafa vuruşu yapmak iste-miş. Ayaklarının ön bölümü yerle temas ederken uygulamak istediği bu atak sonrası sol ayak baş parmağının geriye zorlandığını hissederek oyuna devam edememiş. Olgu yaralanmadan iki gün sonra Acil Polikliniğimize baş vurdu (18.8.1994 - 11742). Yapılan klinik muayenede sol ayak baş parmağın distal falanksının inter-falangeal eklem seviyesinden dorsale yer değiştirdiği saptandı. Radyolojik değerlendirmede, sol ayak baş parmağma ait interfalangeal eklemde sesamoid kemik interpozisyonuyla aynı eklemde dislokasyon görüldü. Başarısız kapalı redaksiyon girişimlerini takiben açık redüksiyon uygulandı. Dorsal insizyonla artrotomi ya-pıldı. İnterfalangeal eklemde interpoze durumdaki sesamoid kemiğin redaksiyonu sonrası eklemin normal di-zilimi oluşturuldu. Postoperatif dönemde 4 hafta süreyle diz altı alçı atel uygulandı. Olgunun yaralanmadan 22 ay sonra yapılan kontrolünde, klinik olarak herhangi bir şikayetinin ve radyolojik olarak interfalangeal eklemde artritik değişikliklerin olmadığı saptandı.
