Management of infected total knee arthroplasty

Abstract

The outcome of primary total knee arthroplasty (TKA) has improved because of a better understanding of biomechanics and techniques. However while there has been a gradual improvement of the results, there were still some problems. The most difficult and costly cause of the failures is infection. We report a series of 17 revision arthroplasties in 16 patients for infected knee prosthesis. There were 15 women and one man patient. The mean age was 65.2 years and the mean follow-up time was 19.7 months. The outcome of the TKA was assessed using the Knee Society rating score. The knee score improved from a preoperative value of 53.4 points to 81.4 points and the function score improved from 29.3 points to 61.3 points at last evaluation. The failure rates in eradicating infection were 66 % in debridman plus antibiotic therapy and 50 % in one stage reimplantation. But two-stage reimplantation had on overall success rate of 86 % in eradicating infection. We recommend two-stage technique for the management of infected knee arthroplasties except some selected patients.
