Comparison of magnetic resonance imaging to magnetic resonance arthrography in the diagnosis of lateral ligamentous injuries of the ankle

Abstract

Sprains of the lateral ankle ligaments are among the most common sports injuries. Grade 1 or 2 injuries of the lateral ankle ligaments are managed conservatively. However, the treatment of grade 3 injuries is still controversial. Documentation or exclusion of any minor abnormalities around the ankle joint are important before planning a method of treatment. The magnetic resonance arthrography (MRA) in detecting the ligamentous injuries of the ankle. 25 patients (9 men, 16 women) with an inversion sprain to the ankle were included in the study. Average age was 29 years (range, 13-55 years). Anterolateral joint line tenderness and edema was found in almost all of the cases. Anterior drawer test was 2 (+) in 14 of the cases. After excluding any osseous abnormality by routine AP and lateral x-rays, MRI was performed. T1 weighted axial and paracoronal, T2 weighted sagittal images were obtained to evaluate the ligaments of the ankle. Then, 10 to 12 cc. of gadolinium-DTPA in a concentration of 2mMol was injected to the ankle and the same images were obtained again. Anterior talofibular ligament (ATFL) was found to be ruptured in ten of the 11 acute cases and in all of the chronic cases (14 cases). Six of the acute tears and all of the chronic tears could only be diagnosed only with MRA. Two of 6 acute anterior inferior tibiofibular ligament (AITFL) tears were also diagnosed with MRA, but all of 3 chronic AITFL tears were diagnosed with MRI. Additionally, 6 anterolateral hypointense masses related anterolateral impingement, 2 talar bone bruises and 3 talar ostechondritis dissecans was detected. We concluded that MR arthrography may be a useful technique in the diagnosis of ligamentous lesions of the ankle. Especially in cases in which surgery is considered, MRA can give excellent information about the ligamentous, cartilaginous and other structures of the joint and thus may facilitate the decision making.
