Comparison of soft and hard synthetic cast immobilization for the treatment of grade 3 inversion injuries of the ankle
Abstract
In this prospective study, 48 patients with acute grade 3 ankle inversion injuries were randomized to be treated either with soft or hard synthetic cast immobilization. Mean cast immobilization time with full weight bearing was 14 days for both groups and physiotherapy for strengthening and range of motion exercises were started thereafter. The patients were evaluated for instability, pain, limp and swelling at two and six weeks. A questionnaire regarding comfort in cast was also filled by the patients at two weeks. Although soft cast group could walk with less limp and had better range of motion immediately after cast removal, this difference was lost at six weeks. There was no significant difference in terms of instability, pain and swelling at two and six weeks between two groups. During cast treatment patients with soft cast had less restrictions with the activities of daily living, needed less support for walking and returned earlier to their jobs. In conclusion, immobilization with a soft synthetic cast is at least as effective as a hard cast and patient comfort is superior and return to job is earlier.
