Acute septic arthritis of the hip in childhood

Abstract

Fifty-two children with acute septic arthritis of the hip treated from 1976 until 1996 in our clinic were reviewed. Patients were evaluated due to age, infectious agent, duration of symptoms and sequelaes. Joint aspiration material seems to be the most accurate diagnostic parameter. Spesific diagnostic criterias for each age group were determined. The outcome was satisfactory in cases treated within four days of symptoms. Worse prognosis was prominent in cases with sequelaes like septic dislocation and proximal femoral osteomyelitis. Staphylococcus aureus infected hips were having poorer prognosis than other bacteria species. The most important prognostic criterias are age and duration between the initial symptoms and the surgical treatment. An inevitable hip joint destruction could be prevented by early diagnosis, prompt surgical drainage and use of appropriate antibiotics.

Çocukluk cağında akut kalça septik artriti

Öz

1976-1996 yılları arasında kliniğimize başvuran ve kalçada septik artrit tanısı koyularak tedavi edilen 52 hasta çeşitli yönleri ile incelendi. Hastalar yaş grupları, enfeksiyöz ajan, tedavi zamanı ve sökeller açısından değerlendirildi. Tanıda en belirleyici faktörün eklem mayi incelemesi olduğu saptanırken, her yaş grubuna özgü klinik ve laboratuar bulgular belirlendi, ilk 4 gün içinde tedavi edilen hastalarda başarılı sonuçlar alındığı saptandı. Septik çıkık veya proksimal femoral osteomyelit gelişen hastalarda prognoz daha da kötüydü. Staphylococcus aureus ile oluşan enfeksiyonlarda prognoz diğerlerine göre daha kötüydü. En önemli prognostik faktörün yaş ve semptomların başlangıç zamanı ile tedavi arası süre olduğu görüldü. Akut kalça septik artritinde tanının erken konulmasıyla yapılan cerrahi drenaj, uygun antibiyotik tedavisiyle olası bir kalça harabiyetinin önlendiği saptandı.
