Mosaicplasty procedure and fascia lata grafting method for the treatment of isolated osteochondral lesions (Experimental study in sheep)

Mosaicplasty procedure and fascia lata grafting method for the treatment of isolated osteochondral lesions (Experimental study in sheep)

Abstract

Articular cartilage has a limited capacity for repair.We investigated the effect of mosaieplasty and fascia lata grafting method on the healing of full-thickness osteochondral defects in adult Turkish(Karaman) sheep.Double defects,ten milimeters wide by ten milimeters deeps,were created in the medial and lateral condyles of the right femur in twenty sheep. The defects were either left empty on left knees medial condyl,filled with fascia lata grafting tecnique on the right knees lateral condyles or filled with mosaieplasty on the medial condyls of the right knees. The animals were killed at two,four,six or twelve months and the repair tissue were examined arthroscopically, radiologically, macroscopically and evaluated with use of a grading scale by macroscopically described by O'Driscoll. Treatment with mosaieplasty accelerated the formation of new subchondral bone and improve the histological apperance of overlying articuler cartilage.At twelve months the apperance of the repair cartilage was nearly normal adjecent cartilage and a new tidemark usually had formed between repair cartilage and underlying subchondral bone. The total healing were significantly better for the defects treated with mosaieplasty than for the treated with fascia lata grafts at all time-points.The untreated defects demonstrated minimum healing capacity with investigating methods. We described the capacity of mosaieplasty to accelerate the healing of full- thickness defects of articular cartilage and to improve the histological appearance and macroscopical characteristics of the repair cartilage.These improvements were evident as long as twelve months postoperatively in adult sheep.Because of the technical simplicity of mosaieplasty compared other soft tissue grafts and other technical demanding techniques and because of the improvement in healing afforded by mosaieplasty, the use of this grafts to influence the healing of defects of articular cartilage should be investigated further.
