Arthrodiatasis with Ilizarov fixator for treatment of Legg-Calve-Perthes disease

Abstract

Eleven hips of 11 patients with a diagnosis of Perthes' disease, were distracted using an Ilizarov type external fixator between 1993 and 1995. There were 8 boys and 3 girls. Mean age at the onset of the symptoms was 6.9 years (range 5-10 years). The onset of symptoms had occurred an average of 8.2 months before the treatment was begun. 6 children were 8 years old or older at the operation, with a mean age of 7.4 years (range 5-10 years). According to the lateral pillar classification of Herring, 4 of the 11 hips were class B and 7 of class C ; 3 hips were Catterall group 3 and 8 group 4. All patients had one or more poor prognostic signs of Catterall. Eleven patients were followed up for a mean period of 36 months (range 24-50 months). Average application period of the fixator was 93 days (range 40-150 days). Approximately 3 weeks after the application of the fixator, ossification was identified in the lateral half of the epiphysis and an accelerated advancement to Waldenström's reossification phase was noted. During distraction, full containment was achieved in only 6 patients, while was not in the other 5. After removal, containment achieved through the fixator was lost in two more patients. End results according to Stulberg classification were group II for 3 patients, group III for 7 and group IV for one. The most common complication was pin track infection, which occurred in 8 of 11 patients. Although rapid progression to healing phase and the ability to reduce a serious subluxation are promising, the relatively high rate of complications and preliminary results do not justify the routine use of this technique.

Öz

1993-1995 yılları arasında Perthes hastalığı tanısı olan 8'i erkek 3’ü kız toplam 11 hastanın 11 kalçası Ilizarov tipi eksternal fiksatör ile distrakte edildi. Semptomların ortalama başlama yaşı 6-9 yaş (5-10) idi. Semptomlar tedaviden ortalama 8.2 ay önce başlamıştı. 6 çocuk cerrahi sırasında 8 veya daha büyük yaşta, ortalama 7.4 yasta (5-10 yıl ) idiler. Herring'in lateral pillar sınıflamasına göre 11 kalçadan 4'ü B, 7'sı C sınıfı idi: 3 kalça Catterall grup 3, 8 kalça grup 4 idi. Tüm hastalarda Catterall'in bir veya birden fazla kötü prognostik bulgusu vardı. 77 hasta ortalama 36 ay (24-50 ay) takip edildi. Ortalama fiksatör uygulama süresi 93 gündü (40-150 gün). Fiksatör uygulamasında ortalama 3 hafta sonra epifizin lateral yarısında ossifikasyon gözlendi ve hızla Waldenström'ün reossifikasyon fazına geçiş saptandı. Distraksiyon sırasında tam uyum (containment), sadece 6 hastada saptanırken, 5 hastada ise saptanmadı. Fiksatörle elde edilen uyum, fiksatör çıkarılmasını takiben 2 hastada kaybedildi. Stutberg sınıflamasına göre 3 hastada grup II, 7 hastada grup III, l hastada grup IV sonuçları elde edildi. En sık komplikasyon 11 hastadan 8'inde rastladığımız çivi dibi enfeksiyonu idi. Sonuç olarak, iyileşme fazına hızlı geçiş, ciddi subluklasyonların redükte edilebilmesi gibi avantajlarına rağmen, yüksek komplikasyon oranı ve ön sonuçlar bu tekniğin rutin kullanımının henüz desteklememektedir.
