Allografts in cruciate ligament surgery

Autografts and allogrefts are the most commonly chosen surgical materials in reconstruction of cruciate ligaments. The advantages and disadvantages of allografts are discussed in this article.There are 4 basic subjects that we must know before using an allograft material. These are; transmission of disease through transplantation of musculoskeletal allografts, harvesting, preservation and secondary sterilization of allografts, remodeling period of allografts inside the knee joint and immunologic response to allografts. To our current knowledge the risk of disease transmission is very low if the strict rules of tissue banking are followed in harvesting of musculoskeletal allografts although secondary sterilization is not performed. The remodeling period of autografts and allografts in the knee joint are similar and the immunologic response to allograf has no effect on clinical outcomes. The clinical studies that were done by autografts and allografts have approximately same results. Availability of different thickness and length, and shortening the operation time are among the advantages of this expensive graft material.

Çapraz bağ cerrahisinde allogreftler

Öz

Çapraz bağ rekonstrüksüyon cerrahisinde otogreftler ve allogreftler günümüzde en çok kullanılan greftterdir. Bu derlemede allogreftler incelenmiş, avantaj ve dezavantajları üzerinde durulmuştur. Allogreftler üzerinde tartışılan ve kullanılmadan önce fikir sahibi olunması gereken 4 önemli konu vardır. Bunlar; allogreftler ile vericiden alıcıya hastalık transferi, allogreftlerin vericiden alınması, korunması ve sekonder strerilizasyonu, allogreft dokusunun diz içerisinde remodele oluş süreci ve allogreftlere karşı alıcı tarafından geliştirilen immünolojik yanıt konularıdır. Bugünkü bilgilerimiz greft alımı sırasında gereken kurallar uygulandığı taktirde sekonder bir sterilizasyon işlemine gerek kalmadan çapraz bağ cerrahisinde kullanılan allogreftler ile alıcıya hastalık transfer riskinin çok düşük olduğunu göstermektedir. Ayrıca allogreftlerle otogreftlerin diz içerisinde remodele oluş süreçlerinin benzer olduğu, allogreftlere geliştirilen immünolojik yanıtın klinik sonuçlara bir etkisi olmadığı ve allogreftlerle yapılan klinik çalışmalarda otogreftler kadar başarılı sonuçlar elde edildiği gösterilmiştir. Kullanılacak greftin kalınlığı ve boyu konusunda geniş seçenekler sunması, operasyon süresini kısaltması da, ekonomik olarak maliyeti yüksek olan allogreftlerin avantajları arasında sayılır.
