The efect of the high tibial osteotomy on bone density

Abstract

In order to investigate the effect of high tibial osteotomy (HTO) on the bone density; pre-operative and post-operative Dual X-ray Photon Absorptiometry (DEXA) analysis has been used.Bone density is measured in both knees in coronal plan with DEXA Analysis before HTO in 9 patients with osteoarthiritic knee joints (mean age: 60.3) and 7 months after HTO.One ROI is selected on each side; medial and lateral parts of proximal tibia, and distal femur. Unoperated-contralateral side measurements are used. as controls The average bone mineral density which is found to be 0.86 gr/cm2 in medial condyle of femur before HTO increased up to 0.95 gr/cm2 after HTO. In medial tibial plateau,it increased from 0.82 gr/cm2 to 0.98 gr/cm2. On the contralateral side;these measurements increased to 1.11 gr/cm2 and 1.07 gr/cm2 and 1.07 gr/cm2 . On the operated side; the bone density of all compartments except medial femoral condyle increased statistically significant. The increase in all the compartments of the contralateral unoperated side also found to be statistically significant. On the operated knee the ratio of the bone densit of the medial femoral condyle to the lateral is found to be increased. After HTO, the bone density of the bony structures around the knee joint increases significantly and the contralateral side.

Yüksek tibial osteotominin kemik yoğunluğuna etkisi

Öz

Gonartozlu olgularda uygulanan yüksek tibial osteotomi (YTO)'nin kemik yoğunluğu üzerindeki etkisini arattırmak amacı ile preoperatif ve postoperatif Dual X-ray Photon Absorptiometry" (DEXA) analizi kulla-nıldı. Yaş ortalaması 60.3 olan 9 gonartrozlu olguda YTO öncesinde ve YTO dan ortalama 9.7 ay sonra her iki dizin koronal planda DEXA analizi ile kemik yoğunluğu değerlendirdi. Olguların tibia proximalinde ve femur distalinde medialde ve lateralde birer adet ilgi alanı seçildi. Karşı taraf ölçümleri kontrol grupları olarak kullanıldı. YTO öncesinde femur medial kondilinde ortalama 0.86 gr/cm2 olan kemik yoğunluğu, YTO sonrasında 0.95 gr/cm2 ye, medial tibia platosunda ise 0.82 gr/cm2 den 0.98gr/cm2 ye artış gösterdi. Karşı tarafta da, bu değerler 0.96 ve 0.92 gr/cm2 den 1.11 ve 1.07 gr/cm2 ye artış gösterdi. Opere tarafta, femur medial kandili dışındaki tüm kompanmanlarda kemik yoğunluğu değerleri istatistiksel olarak anlamlı arnş gösterdi. Opere olmayan tarafta da tüm kompartmanlardaki artiş, istatistiksel olarak anlamlı idi. Opere olan dizde, femur medial kondilindeki kemik yoğunlugunun lateral kondilinkine oranı YTO sonrası artış gösterdi. YTO sonrasında, diz çevresi kemik yapılarında kemik yoğunluğu anlamlı bir artış göstermekte: buna karşı tarafta da bir artış eşlik etmektedir.
