Talectomy for equinovarus deformity in arthrogryposis and meningomyelocele

Abstract

Equinovarus deformity of the foot in arthrogryposis and myelomeningocele is usually resistant to manipulative methods of treatment and often recur after traditional operations such as soft - tissue releases and tendon elongations. Pantalar arthrodesis has been reported unsuccessful but triple arthrodesis was reported satisfactory in the treatment of such deformities. The necessity of waiting for the foot’s maturation until 10-12 years is the drawback of triple arthrodesis. Talectomy was carried out for the treatment of severe clubfoot deformity of 16 feet in 10 patients with arthrogryposis or myelomeningocele between 1995 and 1998 in our clinic. The age at surgery ranged from 8 months to 3 years and 4 months with an average of 16 months. Our average followup length is 22 months. In 56.25 % of the patients, a plantigrade foot was achieved without recurrence. A second posterior release was necessary in 18.75% of patients. In the followup, these patients’ results were also evaluated as satisfactory. In 4 feet, there were unsatisfactory results (25 %). We think these are associated with untreated muscle imbalance. We recommend talectomy before the expected age of walking for the treatment of severe clubfoot deformity in arthrogrypotic and myelodisplastic patients.
