Thumb duplication and its surgical treatment

Abstract

Thumb duplication is the most common type of preaxial polydactyly. In our clinic, 29 patients have undergone surgical correction of such deformities. According to the Wassel classification, it has been noted that the most of the deformities were in the Wassel Type IV which is followed by Type II. Wassel Type VII (triphalangism) is not included in this series. Surgical correction should not only be excising the extra digit, but soft tissue and skeletal structure reconstruction should be performed in order to obtain a functional and aesthetic thumb. According to the type of Wassel classification, surgical treatment was done by simple excision, excision and collateral ligament reconstruction, excision- collateral ligament,tendon,intrensec reconstructions and osteotomies, and Bilhout-Cloquet operation. Secondary deformities after the surgery are frequent in this type of congenital hand anomaly. Angulation, Z-deformity, broad thumb and split nail deformities are the secondary deformities in our series.

Başparmak duplikasyonu ve cerrahi tedavisi

Öz

Duplike veya çift başparmak deformitesi preaksiyel polidaktilinin en sık görülen tipidir. Kliniğimizde bu tip konjenital el anomalisi olan 29 başparmakla cerrahi tedavi gerçekleştirilmiştir. Duplikasyon Wassel sınıflamasına göre gruplandırılmış, en sık Wassel Tip IV. ikinci olarak ise Wassel Tip II anomalilere rastlanılmıştır. Wassel Tip VII (trifalangism) bu seri dışında tutulmuştur. Cerrahi tedavi, genellikle radialde fazla olan parmağın eksizyonu şekilinde değil, oldukça komplike yumuşak doku ve iskeletsel yapının rekonstrüksiyonu ile yapılmalıdır. Ancak bu şekilde fonksiyonu tam ve estetik olarak yeterli bir başparmak elde etmek mümkündür. Bu amaçla Wassel sınıflaması tiplerine göre eksizyon, eksizyon ve kollateral ligament rekonstrüksiyonu, eksizyon- kollateral ligament, tendon, interensek rekonstrüksiyon ve osteotomiler, Bilhout-Cloquet ameliyatları yapılmıştır. Bu konjenital anomalide, tedavi sonrasında da oldukça yüksek sayıda sekonder deformiteler görüldüğü unutulmamalıdır. Serimizde sekonder olarak, angulasyon, Z-deformitesi, geniş başparmak ve 'split' tırnak deformiteleri bulunmuştur.
