Pitfalls in the MRI diagnosis of meniscal lesions

Abstract

The most common knee injuries are lesions of the medial and lateral menisci. Meniscal tears are still diagnostic challenge. Nowadays, MRI is frequently being ordered by orthopaedic surgeons. Sixty-five patients with suspected meniscal lesions prospectively investigated between December 1995 and September 1997. Before arthroscopy, each patient had MRI scans taken. 13 false interpretation (%20) were obtained on MRI. 9 false positive (%14) and 4 false negative (%6) results were present. The overall values for MRI medial and lateral menisci combined were accuracy 76%, sensitivity 90%, specificity 47%, negative predictive value 66% and positive predictive value 84%. 12 of the 13 false interpretations were in the medial meniscus. We concluded that most of the MRI errors are due to increase in signal intensity within the posterior third of the medial meniscus. This is due to myxoid degeneration. On the basis of these errors in MRI, we suggest that clinical assessment should be done very carefully and MRI should not be overused in the evaluation of meniscal tears.
