Peroperative flap monitorization with fluorescein in limb salvage operations
Abstract
Large soft tissue dissection and bone resection may be necessary in the limb sparing surgery and as a result, skin necrosis may develop in the postoperative period. The aim of this study is to investigate the efficacy of peroperative fluorescein angiography in prediction postoperative skin necrosis. Between June 1993 and April 1997, a prospective study is performed in randomly selected 28 tumor patients. Twelve of the patients were osteosarcoma, 3 were Ewing sarcoma. The rest of them was consist of different tumor categories such as aggresive fibromatosis, giant cell tumor or other malignant type tumors. Fluorescein infusion was given (Cibaviscon ophtalmics 0.5/5 mg % 10 - 400 cc Serum physiologic) after completion of the resection of the tumor. Infusion solution were applied dosage of 15 mg/kg via intravenous route. Infusion rate was 10 drops per minute. After 30 minutes, incisional skin flaps were examined with naked eye peroperatively under ultraviolet light (3600 A). Skin margin, with blue and dark green colour was recorded as suspected area. Four deep and 3 superficial skin necrosis were determined. All patients with skin necrosis had suspected area greater than 1 cm. No false positive and negative results was encountered. Relationship between various parameters such as tumor diameter, amount of resection and skin necrosis. Results were found to be statistically insignificant. As a results; fluorescein can safely be used in the limb salvage operation peroperatively without any complication. This monitorisation method is an effective way to predict the skin necrosis.
