Comparison of the functional evaluation methods in rat sciatic nerve model by a new system

Abstract

Objectives: Siatic functional index (SFI) and ankle stance angle (ASA) are two methods commonly used in the evaluation of functional nerve recovery in rat sciatic nerve models. In this study we designed a new walking track by which synchronous SFI and ASA measurements were made, evaluating the relationship between these measurements and functional nerve recovery. Methods: Twenty-one rats were divided into three groups: the sciatic nerve was crushed in the first group, transected in the second, and in the third group, it was first cut then repaired by the epineural technique. Three months later the rats were allowed to walk on the track, with a mirror beneath, which made it possible to synchronously record the step cycles and the images of the sole reflected on the mirror. Images were transmitted to the computer and SFI and ASA were measured from the data obtained. Siatic functional index was measured using the classical method as well. Results: Because of foot problems resulting from severe nerve injury, it was impossible to measure SFI in four rats in group II and in two rats in group III, whereas ASA was measured in all rats. There was no difference between SFI values obtained by classical and new methods (p>0.05). Both methods showed that the best functional results and the worst functional recovery were obtained in group I and group II, respectively. Group III rats exhibited a significant improvement by SFI measurements (p<0.05), but this could not be demonstrated by ASA values (p>0.05). Conclusion: The study showed that it was possible to measure SFI and ASA synchronously by the new walking-track, that severe nerve injuries might lead to some data loss in SFI measurements, and that there was no complete correlation between SFI and ABDA in the evaluation of functional nerve recovery.

Sıçan siyatik sinir modelinde fonksiyonel değerlendirme yöntemlerinin yeni bir sistemle karşılaştırılması

Öz

Amaç: Siyatik fonksiyonel indeks (SFİ) ve ayak bilek duruş açısı (ABDA), sıçan siyatik sinir modelinde, fonksiyonel sinir iyileşmesinin değerlendirilmesinde yaygın olarak kullanılan ölçümlerdir. Bu çalışmada, yeni bir yürüme koridoru sistemi oluşturarak eşzamanlı SFİ ve ABDA ölçümleri yapmayı ve bu ölçümlerin fonksiyonel sinir iyileşmesiyle olan ilişkisini araştırmayı amaçladık. Çalışma planı: Yirmi bir adet sıçan üç gruba ayrıldı: Birinci grupta siyatik sinirde ezilme yaralanması oluşturuldu; ikinci grupta siyatik sinir kesilip bırakıldı; üçüncü grupta ise siyatik sinir kesildikten sonra epinöral teknikle dikildi. Sıçanlar üç ay sonra, altında ayna sistemi bulunan yürüme koridorunda yürütüldü. Yeni sistem sıçanların adım sikluslarının tüm fazlarının ve ayna üzerinden yansıyan ayak tabanı görüntülerinin eşzamanlı olarak video kameraya aktarımına olanak verdi. Görüntülerin bilgisayara aktarılmasıyla elde edilen verilerden SFİ ve ABDA ölçümleri yapıldı. Siyatik fonksiyonel indeks klasik ayak izi yöntemiyle de hesaplandı.. Bulgular: Şiddetli sinir zedelenmesi durumunda gelişen ayak problemleri nedeniyle grup II’den dört, grup III’ten iki sıçanın SFİ ölçümleri yapılamadı; ABDA ölçümleri tüm sıçanlarda yapıldı. Klasik ve yeni yöntemle hesaplanan SFİ ölçümleri arasında fark saptanmadı (p>0.05). En iyi fonksiyonel düzelmenin birinci grupta, en kötü fonksiyonel geri dönüşün ise ikinci grupta olduğu her iki ölçüm yöntemiyle gösterildi. Üçüncü gruptaki sıçanların SFİ ölçümlerinde istatistiksel olarak anlamlı bir iyileşme saptanırken (p<0.05), bu durum ABDA ölçümlerine yansımadı (p>0.05). Çıkarımlar: Yeni yürüme koridoru SFİ ve ABDA ölçümlerinin eşzamanlı yapılabilmesini sağladı. Şiddetli sinir zedelenmesinde SFİ ölçümlerinde veri kaybı oldu. Fonksiyonel sinir iyileşmesinin değerlendirilmesinde SFİ ve ABDA ölçümleri arasında tam bir korelasyon saptanmadı.
