Radiological features of pes cavus deformity

Abstract

Objectives: To analyze the relationships between footprints, local examination findings, and radiological measurements in pes cavus and to define the most appropriate radiologic parameter in the evaluation of this deformity. Methods: Footprints of 3,511 primary school children (mean age 10 years) in Isparta, were screened and 406 children were found to have pes cavus deformity, at least in one foot. Detailed local examinations were performed and footprints were evaluated with the use of podoscope. Weight bearing X-rays were obtained in 261 patients. The results were compared with those of a control group consisting of 42 age-matched normal children. Radiological findings obtained from 606 feet were evaluated in comparison with age and local examination findings. Results: Radiological measurements did not differ significantly between feet having 1st, 2nd, and 3rd degree pes cavus footprints, whereas measurements of feet with 4th degree pes cavus footprints differed significantly. The modified first metatarsal inclination angle, Hibbs’ and Meary angles showed increases, and the lateral talocalcaneal angle showed decreases with age. The other angles were not found in correlation with age. Hibbs’ angle and the inferior cortex of calcaneus-first metatarsal angle decreased, and all the other angles increased with the severity of the deformity. In addition, calcaneus width/length ratio was significantly higher in cases with pes cavus (p=0.004). Of all measurements, Hibbs’ angle showed the highest correlation with the severity of the deformity (p=0.000). Conclusion: Hibbs’ angle seems to be the most instrumental radiological parameter in pes cavus deformity.
